Continuous quality improvement (CQI) training helps private primary care clinics improve preventive services. This study shows clinics will voluntarily engage in CQI to enhance care processes.

Area of Science:

  • Health Services Research
  • Quality Improvement Science
  • Primary Care Medicine

Background:

  • Clinical practice guidelines for preventive services are often not implemented effectively.
  • Existing models for developing and sustaining office systems for change are lacking, especially in non-academic settings.

Purpose of the Study:

  • To test if HMO sponsorship of a continuous quality improvement (CQI) intervention can lead to sustained organizational change and improved preventive service rates in private primary care clinics.
  • To evaluate the implementation and maintenance of office systems within these clinics.

Main Methods:

  • The IMPROVE study is a large randomized controlled trial involving 44 private primary care clinics.
  • Intervention clinics received training, consultation, and support for internal teams to improve preventive service delivery.

Related Experiment Videos

  • Outcomes measured include rates and quality of eight adult preventive services, compared to matched control clinics.
  • Main Results:

    • All 44 clinics were recruited and randomized, with no baseline differences between groups.
    • After nine months, 21 of 22 intervention clinics completed training and initiated systematic improvement processes.
    • Early engagement suggests voluntary participation in CQI efforts is feasible.

    Conclusions:

    • Private primary care clinics can be motivated to undertake systematic quality improvement for preventive services.
    • External training and consultation are key to engaging clinics in multidisciplinary CQI efforts.
    • This approach shows promise for improving the delivery of preventive care in diverse practice settings.
