42K exchange during myocardial ischemia

K I Shine, A M Douglas, N Ricchiuti

    The American Journal of Physiology

    Summary

Rabbit heart septa showed no persistent sodium-potassium pump impairment after 45 minutes of ischemia. Longer ischemia (60 minutes) led to potassium loss and irreversible injury, impacting mechanical function.

    Area of Science:

    • Cardiovascular Physiology
    • Cellular Metabolism
    • Biophysics

    Background:

    • Understanding cellular ion transport is crucial for cardiac health.
    • Ischemia-induced cellular damage affects ion homeostasis and pump function.
    • Potassium (K) exchange dynamics provide insights into cell membrane integrity.

    Purpose of the Study:

    • To investigate potassium-42 (42K) exchange in rabbit interventricular septa before and after total ischemia.
    • To assess the impact of varying ischemia durations on 42K content and exchange rates.
    • To correlate changes in 42K dynamics with recovery of mechanical function and cellular injury.

    Main Methods:

    • Isolated, blood-perfused rabbit interventricular septa were used.
    • 42K labeling was performed to asymptotic values.
    • Total ischemia was induced for durations up to 60 minutes.
    • Tissue probe counts and effluent analysis measured 42K content and exchange rate constants during ischemia and reperfusion.

    Main Results:

    • No persistent impairment of Na pump activity was observed after 30-45 minutes of total ischemia.
    • Septa reperfused after 45 minutes or less of ischemia showed preserved 42K content, with some increases upon reperfusion.
    • Ischemia exceeding 45 minutes led to progressive 42K losses.
    • Decreased 42K exchange rate constants were observed, indicating potential inhomogeneity.
    • After 60 minutes of ischemia, irreversible mechanical injury correlated with significant tissue potassium loss.

    Conclusions:

    • Short durations of total ischemia (≤45 minutes) do not cause persistent impairment of Na pump activity in rabbit septa.
    • Longer ischemia (≥60 minutes) results in significant potassium loss and irreversible mechanical injury.
    • Recovery of mechanical function is not consistently linked to 42K preservation after short ischemia durations.
