Microstomia, a small mouth, can occur without progressive systemic sclerosis. Long-term follow-up of two patients confirmed no scleroderma development, despite other autoimmune conditions.

Area of Science:

  • Rheumatology
  • Dermatology
  • Internal Medicine

Background:

  • Microstomia is a potential sign of connective tissue diseases.
  • Progressive systemic sclerosis (scleroderma) is characterized by microstomia.

Observation:

  • Two patients presented with microstomia but lacked other signs of progressive systemic sclerosis.
  • Patient 1 had systemic lupus erythematosus.
  • Patient 2 had rheumatoid arthritis and Sjögren's syndrome with myositis.

Findings:

  • Neither patient developed features of progressive systemic sclerosis over nearly two decades of follow-up.
  • The presence of microstomia alone did not predict scleroderma development in these cases.

Implications:

  • Microstomia in the absence of other scleroderma indicators may not necessitate specific scleroderma surveillance.
  • This finding expands the differential diagnosis for microstomia in patients with autoimmune diseases.
