Summary

Local recurrence of breast cancer after conserving surgery often happens in the same breast segment. New surgical techniques removing the tumor and associated ducts may reduce recurrence rates.

Background:

  • Local operative therapy for mammary carcinoma is increasingly accepted.
  • Minimizing local recurrence and optimizing cosmetic outcomes are key goals.
  • Intraductal spread is a significant factor in local recurrence after breast-conserving therapy.

Observation:

  • A review of 86 patients with mammary carcinoma treated with local excision identified 5 cases of local recurrence.
  • The local recurrence rate in this series was 5.8%.
  • All recurrent lesions were located in the same breast quadrant as the primary tumor.

Findings:

  • Local recurrence of mammary carcinoma after breast-conserving surgery most frequently occurs within the same anatomical segment.
  • Recurrent lesions were consistently found along a radius from the nipple, crossing the initial lesion site.
  • The location of recurrence suggests a relationship with the ductal system of the primary tumor.

Implications:

  • A primary excision technique removing the dominant tumor mass en bloc with the associated duct system may substantially reduce local recurrence.
  • Understanding the anatomic basis of recurrence can refine surgical strategies for breast-conserving therapy.
  • Further research into duct-oriented excision techniques is warranted to improve outcomes in breast cancer treatment.
