Summary

This study found that changes in cardiac index (CI) directly and immediately affect arterial PCO2 (PaCO2) and end-tidal PCO2 (PETCO2) in heart surgery patients. PETCO2 can reliably estimate CI changes during unsteady states.

Background:

  • A direct relationship between cardiac index (CI) and end-tidal PCO2 (PETCO2) has been previously reported.
  • However, arterial PCO2 (PaCO2) was not measured in prior studies, leaving a gap in understanding the immediate physiological responses.

Purpose of the Study:

  • To investigate the immediate effects of alterations in cardiac index (CI) on arterial PCO2 (PaCO2) and end-tidal PCO2 (PETCO2).
  • To analyze the relationship between PaCO2 and PETCO2 (Pa-PETCO2) in response to changes in CI.

Main Methods:

  • Measurements of CI, PaCO2, and PETCO2 were performed in 20 patients undergoing elective heart surgery before and after sternotomy.
  • Standard thermodilution, infra-red analysis, and blood gas analysis were used.

  • Linear regression analysis was employed to evaluate the relationships.
  • Main Results:

    • Significant, direct, and immediate correlations were observed between CI and both PETCO2 (r=0.75, P<0.001) and PaCO2 (r=0.64, P<0.005).
    • The ratios of change were 3.8 mmHg L⁻¹ for PETCO2 and 4.2 mmHg L⁻¹ for PaCO2.
    • Individual variations in PCO2 changes led to variable Pa-PETCO2 differences, unrelated to CI changes.

    Conclusions:

    • The findings explain the wide variations in Pa-PETCO2 observed with cardiac output fluctuations.
    • These observations are specific to the unsteady physiological state.
    • End-tidal PCO2 (PETCO2) shows potential as a reliable non-invasive indicator for estimating changes in cardiac index (CI).
