High fasting plasma insulin (FPI) levels are linked to ventricular premature complexes (VPCs) in individuals without diabetes or heart disease. This association suggests insulin resistance may play a role in cardiac arrhythmias.

Background:

  • Fasting plasma insulin (FPI) is a marker of insulin resistance.
  • Ventricular premature complexes (VPCs) are common cardiac arrhythmias.
  • The relationship between FPI and VPCs in individuals without traditional risk factors is not well understood.

Purpose of the Study:

  • To investigate the association between fasting plasma insulin (FPI) levels and ventricular premature complexes (VPCs).
  • To explore this association in a cohort free of coronary artery disease, diabetes, and hypertension.

Main Methods:

  • 186 subjects without CAD, diabetes, or hypertension underwent 24-hour ECG monitoring and oral glucose tolerance testing.
  • Fasting plasma insulin, lipids, triglycerides, and potassium were measured.

  • Statistical analyses included correlation and multiple logistic regression.
  • Main Results:

    • Higher FPI tertiles were associated with older age, higher BMI, central obesity, and increased VPCs.
    • FPI concentration independently correlated with VPCs (r = .19, P < .01).
    • The association remained significant after adjusting for multiple factors, but was attenuated by fasting plasma free fatty acids or potassium.

    Conclusions:

    • Fasting plasma insulin concentrations are associated with ventricular premature complexes in subjects without diabetes, hypertension, or ischemic heart disease.
    • Insulin resistance may contribute to cardiac arrhythmias in seemingly healthy individuals.
    • Further research is needed to elucidate the mechanisms linking FPI, metabolic factors, and VPCs.
