Summary

Background:

Cisplatin and carboplatin are platinum-based chemotherapy agents widely used in cancer treatment.
Common side effects include nausea, vomiting, neuropathy, nephrotoxicity, hearing loss, and bone marrow suppression.
Allergic reactions, though rare, can occur with these agents.

Observation:

A young woman undergoing treatment for ovarian cancer experienced an unusual reaction to carboplatin.
The patient presented with acute symptoms including skin rash, dyspnea (shortness of breath), and facial/truncal erythema.
Importantly, hemodynamic stability was maintained, with no significant drop in blood pressure or change in heart rate.

Findings:

This case highlights a unique idiosyncratic hypersensitivity reaction to carboplatin.
The observed symptoms differed from typical anaphylactic responses, particularly the absence of hypotension.
Rapid resolution of symptoms was achieved with the administration of hydrocortisone and promethazine.

Implications:

This case underscores the importance of recognizing diverse hypersensitivity reactions to carboplatin.
It suggests that prompt intervention with corticosteroids and antihistamines can effectively manage such idiosyncratic reactions.
Further investigation into the mechanisms of carboplatin hypersensitivity may inform safer treatment protocols.
