Eighty-four percent of men with aortoiliac occlusive disease (AIOD) experience erectile dysfunction (ED). Endarterectomy surgery is more effective than reconstruction for improving erectile function (EF) in AIOD patients.

Area of Science:

  • Vascular Surgery
  • Urology
  • Cardiovascular Medicine

Background:

  • Aortoiliac occlusive disease (AIOD) is a significant cause of peripheral artery disease.
  • Erectile dysfunction (ED) is a common comorbidity in patients with AIOD, impacting quality of life.
  • The pathophysiology of ED in AIOD and the impact of surgical interventions on erectile function (EF) require further elucidation.

Purpose of the Study:

  • To determine the incidence and pathophysiology of ED in patients with AIOD.
  • To evaluate the effects of aortofemoral surgery, specifically endarterectomy (E) and reconstruction (R), on EF.
  • To compare the efficacy of endarterectomy versus reconstruction in improving or maintaining EF.

Main Methods:

  • Prospective evaluation of 31 male patients undergoing aortofemoral surgery.

Related Experiment Videos

  • Assessment of erectile function (EF) using questionnaires and penile dynamic Colour Doppler Ultrasonography before and 3 months post-surgery.
  • Categorization of ED causes into arteriogenic, venogenic, combined, and neurogenic components.
  • Main Results:

    • Eighty-four percent (26/31) of patients with AIOD presented with ED.
    • The primary causes of ED were combined arteriogenic and venogenic (53%), purely venogenic (23%), neurogenic (16%), and purely arteriogenic (8%).
    • Post-surgery, 7 out of 20 evaluated patients showed improved EF; 6 of these had undergone endarterectomy (E), while only 1 had reconstruction (R).

    Conclusions:

    • The majority of patients with AIOD suffer from ED, with both reduced penile arterial inflow and veno-occlusive dysfunction contributing significantly to its pathophysiology.
    • Atherosclerosis in AIOD may also impair the penile veno-occlusive mechanism.
    • Endarterectomy appears to be a more favorable surgical option than reconstruction for improving or preserving erectile function in patients with AIOD.
