    Summary

    This report details a rare case of bilateral lung sequestrations in a teenage boy. The unique mirror-image vascularization of both intralobar and extralobar types presents an interesting anatomical variant.

    Background:

    • A 15-year-old male presented with coexistent bilateral intralobar and extralobar pulmonary sequestrations.
    • A distinct mirror-image vascular anatomy was noted for each sequestration.

    Findings:

    • Arterial supply for both sequestrations originated from a common trunk of the thoracic aorta.
    • Venous drainage from both sequestrations converged into a single channel draining into the azygos system.

    Implications:

    • This case highlights an unusual vascular variant of bilateral pulmonary sequestrations.
    • Understanding such unique anatomy is crucial for surgical intervention and patient management in pediatric thoracic surgery.
