Summary

Candida in the gallbladder is a serious concern, especially for critically ill intensive care unit (ICU) patients, who have a 100% mortality rate. Treatment for biliary Candida requires antifungals and broad-spectrum antibiotics.

Area of Science:

  • Medical Microbiology
  • Gastroenterology
  • Infectious Diseases

Background:

  • The clinical significance of Candida species in the gallbladder and biliary tract is not well-established.
  • Understanding the impact of Candida infections in this anatomical region is crucial for patient outcomes.

Purpose of the Study:

  • To investigate the clinical significance of Candida species isolated from the gallbladder and biliary tract.
  • To analyze patient demographics, clinical presentations, treatments, and outcomes associated with biliary Candida isolates.

Main Methods:

  • A retrospective analysis of patients with Candida species isolated from gallbladder and biliary tract sources over a 10-year period.
  • Review of medical records for details on biliary disease, microbiological data, therapeutic interventions, and patient outcomes.

Main Results:

  • Twenty-seven patients were identified with Candida isolated from biliary sources.
  • Candida cholecystitis in critically ill intensive care unit (ICU) patients had a 100% mortality rate.
  • Candida was frequently found in patients with biliary stents, obstruction, and cholangitis; candidemia was uncommon.

Conclusions:

  • Candida cholecystitis in ICU patients carries a grave prognosis.
  • Treatment for biliary Candida, particularly with stents or cholangitis, necessitates antifungal and broad-spectrum antimicrobial agents.
  • Candidemia is infrequently observed in patients with Candida isolated from the biliary tract.
