Progressive systemic sclerosis (PSS) can affect the peripheral nervous system, causing neuropathy. Sural nerve biopsies in PSS patients revealed nerve damage, suggesting an immune-related cause for this rare complication.

Area of Science:

  • Neurology
  • Immunology
  • Pathology

Background:

  • Progressive systemic sclerosis (PSS) is a chronic inflammatory disease primarily affecting skin and visceral organs.
  • Peripheral nervous system involvement is rare in PSS.

Observation:

  • Two female PSS patients without other collagen diseases developed peripheral neuropathy.
  • Neuropathy presented as multiple mononeuropathy in limbs and trunk.

Findings:

  • Sural nerve biopsy showed axonal degeneration, myelin loss, and increased collagen.
  • No signs of vasculitis were observed.
  • Electron microscopy revealed degenerated axons, disrupted myelin, and capillary basal lamina changes.

Implications:

  • Mononeuropathy in PSS may stem from immune-mediated vasculopathy.
  • This suggests an ischemic basis for neuropathy in PSS.
