[Ticlopidine-induced neutropenia]

Background:

Ticlopidine (TCP) is an antiplatelet medication.
TCP is recognized for its efficacy in preventing stroke and thromboembolic diseases, often outperforming aspirin.
A significant adverse effect of TCP is neutropenia, occurring in approximately 2% of patients.

Observation:

This report details two cases of patients hospitalized with severe neutropenia.
Both patients were undergoing treatment with ticlopidine.
Despite regular blood count monitoring, severe neutropenia developed in these individuals.

Findings:

The observed cases highlight that even regular blood count monitoring may not prevent severe neutropenia in all patients on ticlopidine.
Neutropenia associated with ticlopidine can progress to serious conditions like agranulocytosis, sepsis, and death.
These complications underscore the risks associated with ticlopidine therapy.

Implications:

Meticulous patient follow-up and careful patient selection are essential when prescribing ticlopidine.
The potential for severe adverse hematological events necessitates heightened clinical vigilance.
Alternative antiplatelet strategies may be considered for patients at higher risk.
