Summary

Congenital herpes zoster in a newborn, resulting from maternal varicella exposure, showed a benign course. Prompt treatment with varicella zoster immunoglobulin and acyclovir led to complete healing of skin lesions.

Background:

  • Maternal varicella zoster virus (VZV) infection during pregnancy can pose risks to the fetus.
  • Congenital herpes zoster (shingles) is a rare manifestation in newborns exposed to VZV in utero.

Observation:

  • A newborn girl presented with an erythematous maculopapular rash and grouped vesicles on the right L2-L4 dermatome shortly after birth.
  • The mother had a history of varicella infection exposure during the seventh month of gestation.

Findings:

  • The newborn was treated with varicella zoster immunoglobulin and both oral and topical acyclovir.
  • Complete resolution of the skin lesions was observed within eight days of treatment initiation.

Implications:

  • This case highlights the link between maternal VZV exposure and congenital herpes zoster.
  • It underscores the generally benign prognosis and favorable response to acyclovir therapy in neonates with this condition.
