A rare superior cerebellar artery aneurysm was successfully clipped using an orbitozygomatic transsylvian approach. This skull base technique is recommended for high-positioned aneurysms in this location.

Area of Science:

  • Neurosurgery
  • Vascular Neurology
  • Skull Base Surgery

Background:

  • Peripheral superior cerebellar artery aneurysms are rare cerebrovascular diseases.
  • Subarachnoid hemorrhage from these aneurysms presents a significant surgical challenge.
  • The anterior pontomesencephalic segment is a critical location for aneurysm treatment.

Observation:

  • A 48-year-old male presented with subarachnoid hemorrhage due to a superior cerebellar artery aneurysm.
  • The aneurysm's neck was positioned high, 10 mm above the biclinoid line.
  • Initial neurological examination revealed only a stiff neck.

Findings:

  • Computed tomography confirmed subarachnoid hemorrhage.
  • Left vertebral angiography identified the aneurysm at the anterior pontomesencephalic segment of the left superior cerebellar artery.

Related Experiment Videos

  • Surgical clipping was performed via an orbitozygomatic transsylvian approach.
  • Implications:

    • The orbitozygomatic transsylvian approach is effective for treating high-positioned superior cerebellar artery aneurysms.
    • This surgical strategy offers a viable option for complex aneurysm cases.
    • Successful clipping resulted in an uneventful postoperative recovery, highlighting the approach's safety and efficacy.
