Extrahepatic lymph node metastases can occur in small hepatocellular carcinoma (HCC) less than 2 cm. This finding suggests that even small HCC tumors may have a poor long-term prognosis due to widespread metastasis.

  • Extrahepatic lymph node metastases from hepatocellular carcinoma (HCC) are typically associated with advanced disease.

  • A case of a small HCC (18 mm) with a capsule presented with multiple, systemic lymph node metastases.
  • Widespread lymphadenopathy mimicking lymphoma developed two months post-resection.
  • Biopsy confirmed metastatic HCC in a supraclavicular lymph node.

Findings:

  • Small HCCs (<2 cm) can exhibit extrahepatic lymph node metastasis.
  • Metastatic HCC in lymph nodes can occur even with a small primary tumor size.
  • The patient's prognosis was adversely affected by the widespread lymph node metastasis.
