Background:

  • Multiple myeloma, a plasma cell malignancy, can present with unusual extramedullary manifestations.
  • Intracranial involvement in multiple myeloma can be solitary or part of disseminated disease.

Observation:

  • A patient with multiple myeloma, previously treated with chemotherapy, developed sudden neurological deterioration.
  • A large (5-cm) tumor infiltrating the meninges and brain was identified in the left temporal fossa, causing increased intracranial pressure.

Findings:

  • Radiological diagnosis using contrast-enhanced CT and angiography was inconclusive for plasmacytoma due to successful prior chemotherapy for extracranial disease.
  • Differential diagnoses included meningioma, metastasis, lymphoma, chondrosarcoma, and hemangioma.

Implications:

  • This case underscores the importance of considering extraosseous multiple myeloma in the differential diagnosis of intracranial tumors, even after successful treatment.
  • Advanced imaging and potentially biopsy are crucial for accurate diagnosis when radiological findings are ambiguous.
