Area of Science:

  • Anatomy
  • Radiology
  • Orthopedics

Background:

  • Controversy exists regarding iliolumbar ligament anatomy due to lumbosacral region complexity.
  • Previous postmortem studies show variability in ligamentous structures.

Purpose of the Study:

  • To clarify the specific anatomical details of the iliolumbar ligament's insertion.
  • To resolve anatomical questions regarding the iliolumbar ligament.

Main Methods:

  • Magnetic resonance imaging (MRI) was used to analyze 28 iliolumbar ligaments in 14 volunteers.
  • Images were acquired in transversal and coronal planes of the lumbosacral region.

Main Results:

  • The anterior band of the iliolumbar ligament is broad, inserting on the iliac tuberosity.
  • The posterior band is thinner, with a round section, inserting on the iliac crest.

Conclusions:

  • The posterior band's smaller insertion area on the iliac crest may explain its susceptibility to mechanical overload.
  • This could contribute to painful syndromes linked to iliolumbar ligament enthesopathy.
