Background:

  • Ameloblastoma is a common odontogenic tumor, typically benign but with malignant potential.
  • Malignant transformation of ameloblastoma is rare and poorly understood.

Observation:

  • A 63-year-old Japanese female presented with an unusual ameloblastoma in the maxilla.
  • Histopathological examination revealed features of a less-differentiated malignant ameloblastoma.
  • Dendritic cells containing melanin pigment were observed within tumor nests invading bone and soft tissue.

Findings:

  • The tumor exhibited characteristic malignant ameloblastoma features alongside melanocytosis.
  • Melanin granules were present in the cytoplasm of some tumor cells.
  • No melanocytes or melanin pigment were found in the tumor stroma.

Implications:

  • This case represents the first documented instance of melanocytosis in an odontogenic epithelial tumor.
  • The findings suggest a potential link between ameloblastoma and melanocytic differentiation.
  • Further research is needed to elucidate the histogenesis of melanocytes in this context.
