Summary

The interleukin-1 receptor antagonist (IL-1RN) allele 2 is more frequent in ulcerative colitis (UC) patients, particularly those with pouchitis. HLA type and perinuclear anti-neutrophil cytoplasmic antibodies (p-ANCA) status did not significantly impact pouchitis risk.

Background:

  • Previous studies suggest a link between Interleukin-1 receptor antagonist (IL-1RN) variable number tandem repeat (VNTR) allele 2 and ulcerative colitis (UC).
  • The IL-1RN allele 2 has also been associated with increased severity in other inflammatory conditions.
  • Human Leukocyte Antigen (HLA) type and perinuclear anti-neutrophil cytoplasmic antibodies (p-ANCA) have been implicated in UC disease extent.

Purpose of the Study:

  • To investigate the association of IL-1RN allele 2, HLA type, and p-ANCA status with the risk of developing pouchitis in UC patients after restorative proctocolectomy.

Main Methods:

  • Genotyping for IL-1RN and HLA DR beta and DQ beta loci was performed on 28 UC patients without pouchitis and 25 UC patients with pouchitis.
  • IL-1RN genotypes were also assessed in 86 healthy controls and 61 familial adenomatous polyposis (FAP) patients.

    • A significantly increased frequency of IL-1RN allele 2 was observed in UC patients compared to FAP controls, primarily driven by the pouchitis group.
    • No significant variations in HLA haplotype frequencies were found between the groups.
    • p-ANCA status did not show significant associations with pouchitis or correlate with HLA status.

    Conclusions:

    • The IL-1RN allele 2 is associated with an increased risk of pouchitis in ulcerative colitis patients.
    • HLA type and p-ANCA status do not appear to be significant risk factors for pouchitis development in this cohort.
