Summary

Videoscopic-assisted mediastinal lymph node dissection (VATS MLND) shows feasibility and comparable radicality to open surgery in pigs. However, retrotracheal dissection may be less complete with VATS MLND.

Background:

  • Videoscopic-assisted mediastinal lymph node dissection (VATS MLND) is a minimally invasive technique.
  • Comparison of VATS MLND to conventional open surgery is crucial for clinical adoption.

Purpose of the Study:

  • To compare the feasibility, completeness, and morbidity of VATS MLND versus conventional open techniques.
  • To evaluate the efficacy of VATS MLND in mediastinal lymph node dissection in an experimental model.

Main Methods:

  • Ten large white pigs underwent right upper mediastinal lymph node dissection (MLND) with or without upper lobectomy.
  • Six pigs were operated using VATS MLND (group 1), and four using conventional open surgery (group 2).
  • Post-mortem assessment evaluated remaining lymph nodes in the mediastinum.

  • No significant difference in operation time or lymph node yield between VATS MLND and open surgery.
  • No intra- or post-operative complications were observed in either group.
  • Incomplete dissection of retrotracheal lymph nodes was noted in one VATS MLND case.

  • VATS MLND is a feasible and safe procedure with comparable radicality to conventional surgery in paratracheal and peribronchial areas.
  • Conventional open surgery may offer more complete retrotracheal lymph node dissection.
  • Further investigation is warranted for optimizing VATS MLND in specific anatomical regions.
