    Summary

This study compared high-dose penicillin to ticarcillin for complicated urinary tract infections. Both treatments showed comparable results and no significant toxicity in patients with kidney stones undergoing surgery.

    Area of Science:

    • Urology
    • Infectious Diseases
    • Pharmacology

    Background:

    • Complicated urinary tract infections (cUTIs) often require effective antibiotic treatment.
    • Patients with staghorn or multiple calculi undergoing anatrophic nephrolithotomy are at risk for cUTIs.
    • Optimal antibiotic choice for these complex cases is crucial.

    Purpose of the Study:

    • To compare the efficacy and safety of a massive dose of penicillin versus the recommended dose of ticarcillin.
    • To evaluate treatment outcomes in patients with complicated urinary tract infections associated with kidney stones.

    Main Methods:

    • A randomized prospective study involving 50 patients with staghorn or multiple calculi.
    • Patients underwent anatrophic nephrolithotomy and were treated with either massive penicillin (60 million units) or ticarcillin (200 mg/kg).
    • Bacteriologic results and toxicity were monitored over a 14-month period.

    Main Results:

    • Bacteriologic results were comparable between the massive penicillin and ticarcillin treatment groups.
    • No significant toxicity was observed in patients receiving either high-dose penicillin or ticarcillin.
    • Both antibiotic regimens were effective in treating complicated urinary tract infections in this patient cohort.

    Conclusions:

    • Massive penicillin dosage is a viable alternative to ticarcillin for complicated urinary tract infections in patients with kidney stones.
    • Both treatment options demonstrate comparable efficacy and safety profiles.
    • Further research may explore optimal dosing strategies for penicillin in complex UTI cases.
