Area of Science:

  • Pediatric Cardiology
  • Anesthesiology
  • Respiratory Medicine

Background:

  • Cardiopulmonary bypass (CPB) is a critical procedure in pediatric cardiac surgery.
  • Severe bronchospasm is a rare but serious complication during CPB.
  • Asthma history may be a risk factor, though patients may be asymptomatic preoperatively.

Observation:

  • A six-year-old child experienced severe bronchospasm during CPB, specifically when resuming controlled ventilation.
  • The patient had a history of asthma but was asymptomatic before the surgery.
  • Aggressive treatment with multiple bronchodilating agents was required to manage the bronchospasm.

Findings:

  • Severe bronchospasm during CPB is an uncommon event.
  • Diagnosis requires excluding other causes of ventilation failure.

Related Experiment Videos

  • Intravenous bronchodilators are the recommended treatment.
  • Implications:

    • This case highlights the importance of recognizing and managing severe bronchospasm in pediatric CPB.
    • Early and aggressive treatment is essential for successful weaning from CPB.
    • Further research into the unknown etiology of this complication is warranted.
