Summary

Subclinical renal tuberculosis is common in AIDS patients, with 23% showing kidney granulomas. Urine cultures for Mycobacterium tuberculosis can aid in diagnosing this condition in immunocompromised individuals.

Area of Science:

  • Nephrology
  • Infectious Diseases
  • Pathology

Background:

  • Acquired Immunodeficiency Syndrome (AIDS) can lead to opportunistic infections, including tuberculosis.
  • Renal involvement in tuberculosis is often underdiagnosed, especially in immunocompromised patients.
  • Understanding the prevalence of renal tuberculosis in AIDS patients is crucial for timely diagnosis and treatment.

Purpose of the Study:

  • To determine the prevalence of renal tuberculosis in patients with AIDS.
  • To investigate the clinical features associated with renal tuberculosis in this patient group.
  • To evaluate diagnostic methods for renal tuberculosis in AIDS patients.

Main Methods:

  • Analysis of renal tissue from 46 necropsies of patients diagnosed with AIDS.
  • Light microscopy examination for the presence of tuberculous granulomas.
  • Correlation of pathological findings with clinical data, including urinalysis and prior tuberculosis diagnoses.

Main Results:

  • Renal tuberculous granulomas were identified in 11 (23%) of the 46 AIDS patients studied.
  • Three of these patients had no prior diagnosis of renal or extrarenal tuberculosis, indicating subclinical disease.
  • Only 4 patients presented with moderate hematuria or sterile pyuria, highlighting the often asymptomatic nature of renal tuberculosis in this population.

Conclusions:

  • Subclinical renal tuberculosis is frequent in patients with AIDS.
  • Standard urinalysis findings are often absent or non-specific in cases of renal tuberculosis in AIDS patients.
  • Urine culture for Mycobacterium tuberculosis is a valuable diagnostic tool for identifying renal tuberculosis in individuals with AIDS.
