Background:

Osteoporotic vertebral compression fractures are common in the elderly.
Neurologic signs and symptoms can arise from these fractures.
Atypical fluid collections associated with vertebral fractures require characterization.

Purpose of the Study:

To investigate the nature of atypical fluid collections in elderly women with osteoporotic vertebral compression fractures.
To correlate imaging findings with histologic characteristics.

Main Methods:

Magnetic resonance (MR) imaging was performed on three elderly women presenting with neurologic symptoms and osteoporotic vertebral compression fractures.
Computed tomography (CT)-guided biopsies and aspirations were conducted for histologic characterization of the vertebral fluid collections.

MR imaging demonstrated atypical fluid collections in all three patients.
Histologic analysis provided characterization of these vertebral fluid collections.

Conclusions:

The clinical, histologic, and imaging features suggest these fluid collections are most likely associated with underlying avascular necrosis.
This finding provides a potential explanation for neurologic complications in osteoporotic vertebral fractures.
