A basilar artery thrombosis caused stroke-like symptoms and symmetrical thalamic necrosis in a 62-year-old woman. Autopsy revealed thrombosis at the basilar artery bifurcation, leading to characteristic "butterfly type" thalamic and midline brainstem necrosis.
