Clinical experience with retrograde lung preservation

Summary

Retrograde lung preservation is a feasible technique in lung transplantation, enabling simultaneous heart recovery. This method showed promising results with no primary lung graft failure in 21 patients.

Background:

Previous research suggests retrograde lung preservation enhances tracheobronchial wall vascularity.
This technique may improve lung graft viability and surgical outcomes.

Purpose of the Study:

To evaluate the clinical feasibility and initial outcomes of retrograde lung preservation in lung transplantation.
To assess the safety and efficacy of this method for both lung and heart recovery.

Main Methods:

Retrospective analysis of 21 consecutive lung transplant recipients.
Lung preservation via left atrial appendage with pulmonary artery drainage.
Modified Euro-Collins solution for pneumoplegic preservation; standard cardioplegia for heart preservation.

No primary lung graft failure observed in 21 patients.
One instance of bronchial anastomosis dehiscence at 30 days; no stents used.
Satisfactory use of all harvested hearts, except one primary graft failure.
Cardiopulmonary bypass only necessary in three cases of pulmonary hypertension.

Conclusions:

Retrograde lung preservation is a clinically feasible technique for lung transplantation.
Simultaneous heart recovery is achievable with this method.
Further studies are needed to determine the long-term clinical impact of retrograde lung preservation.
