High-dose interferon beta-1b (IFN-beta-1b) therapy significantly improved memory recall in multiple sclerosis (MS) patients. This cognitive benefit in MS was observed without changes in brain lesion area or disability scores.

Background:

  • Multiple sclerosis (MS) is a chronic autoimmune disease affecting the central nervous system.
  • Cognitive dysfunction is a common and debilitating symptom in MS patients.
  • Interferon beta-1b (IFN-beta-1b) is an established treatment for MS, but its effects on cognition require further elucidation.

Purpose of the Study:

  • To longitudinally assess the impact of interferon beta-1b (IFN-beta-1b) on neuropsychological function in patients with multiple sclerosis (MS).
  • To investigate the relationship between cognitive changes, MRI-measured brain lesion area, and clinical disability scores in MS patients treated with IFN-beta-1b.

Main Methods:

  • A longitudinal study involving 30 MS patients from a pivotal trial of IFN-beta-1b.

  • Patients were randomized to high-dose IFN-beta-1b (8.0 million units), low-dose IFN-beta-1b (1.6 MIU), or placebo.
  • Neuropsychological tests (Wechsler Memory Scale), MRI, and Expanded Disability Status Scale (EDSS) were assessed over 4 years.
  • Main Results:

    • Significant improvement in visual memory recall (Wechsler Memory Scale Visual Reproduction-Delayed Recall) was observed in the high-dose IFN-beta-1b group between years 2 and 4.
    • No significant correlation was found between cognitive improvement, motor performance, MRI lesion area, or depression scores.
    • While the high-dose group showed stable lesion area, placebo and low-dose groups exhibited increased lesion area over time.
    • EDSS scores remained stable and did not correlate with MRI lesion area.

    Conclusions:

    • High-dose IFN-beta-1b therapy demonstrates a positive effect on specific aspects of cognitive function, namely delayed visual memory recall, in MS patients.
    • This cognitive improvement is independent of changes in brain lesion burden or overall clinical disability.
    • The findings suggest a potential disease-modifying effect of high-dose IFN-beta-1b on cognitive function in MS, not attributable to practice effects or lesion changes.
