Summary

Background:

Malignant mesothelioma typically originates in the serous membranes, most commonly the pleura or peritoneum.
Primary retroperitoneal tumors are rare, with diverse origins and presentations.

Observation:

A 39-year-old male presented with right flank and scrotal pain, progressing over six months.
Imaging (IVP, CT, ultrasonography) revealed a retroperitoneal tumor obstructing the right ureter, causing kidney dysfunction.
Surgical resection was challenging due to extensive invasion into surrounding muscles and bones.

Findings:

Histopathological examination confirmed the retroperitoneal tumor as a primary malignant mesothelioma.
Post-operative chemotherapy showed no effectiveness against the rapidly progressing tumor.
