    Background:

    • This study describes three patients with co-existing long-standing AS and CES.
    • The neurological symptoms, including radicular pain and genitourinary complaints, developed gradually and were disabling.
    • Myelography revealed dural sac expansion and arachnoidal diverticula.

    Findings:

    • A strong association between ankylosing spondylitis and cauda equina syndrome is highly probable.
    • The pathogenesis likely involves early arachnoiditis leading to reduced meningeal elasticity.

    Implications:

    • Early recognition of nerve root lesions in AS patients is essential.
