Background:

  • Saiboku-to is a traditional Japanese herbal medicine used for various conditions.
  • Adverse drug reactions, including pneumonia, can occur with herbal medications.

Observation:

  • A 72-year-old male patient developed dyspnea and fever 42 days after initiating Saiboku-to therapy.
  • Diagnostic imaging revealed diffuse linear-nodular lung shadows, and blood gas analysis indicated hypoxemia.

Findings:

  • A lymphocyte-stimulation test confirmed Saiboku-to as the causative agent.
  • The patient was diagnosed with Saiboku-to-induced pneumonia.

Implications:

  • Discontinuation of Saiboku-to and administration of prednisolone resulted in marked clinical improvement.
  • This case highlights the importance of considering drug-induced pneumonia in patients on herbal medications and adds to the limited reported cases of Saiboku-to-induced pneumonia.
