Background:

Dermatomyositis is an idiopathic inflammatory myopathy.
Sarcoidosis is a multisystem inflammatory disease characterized by noncaseating granulomas.

Observation:

A 57-year-old woman presented with fever, dry cough, myalgias, and reticular chest X-ray findings.
She exhibited typical dermatomyositis rash and myositis.
Biopsies revealed noncaseating granulomas in salivary gland, muscle, and lung, along with uveitis.

Findings:

Lung biopsy showed interstitial pneumonia, but the presence of granulomas was atypical for dermatomyositis.
Pathological findings confirmed the coexistence of dermatomyositis and sarcoidosis.
The patient's presentation was inconsistent with either disease alone.
