Summary

Mammography can detect axillary abnormalities, most commonly abnormal lymph nodes. While certain features like size and margins suggest malignancy, distinguishing benign from malignant lymph nodes mammographically remains challenging.

Purpose of the Study:

  • To determine the causes and frequency of axillary abnormalities detected mammographically.
  • To evaluate imaging characteristics of lymphadenopathy associated with malignancy.

  • Retrospective review of 96 mammographically detected axillary abnormalities in 94 patients.
  • Correlation with clinical diagnoses and pathological results.
  • Analysis of abnormality length, margins, and microcalcifications using logistic regression.

  • Nonspecific benign lymphadenopathy (29%) and metastatic breast cancer (26%) were most frequent diagnoses.
  • Lymph node length >33mm showed high specificity (97%) for malignancy; ill-defined or spiculated margins were also associated with malignancy.

Conclusions:

  • Abnormal lymph nodes are the most common mammographic axillary finding.
  • Homogeneously dense lymph nodes >33mm with irregular margins or intranodal microcalcifications suggest malignancy.
  • Mammographic distinction between benign and malignant lymph nodes is often difficult.
