    Summary

    Ultrasonic scanning aids in diagnosing biliary duct obstruction after cholecystectomy when other methods fail. However, it should not replace standard cholangiography or invasive procedures when feasible.

    Background:

    • Patients with jaundice and fever post-cholecystectomy require accurate diagnosis of biliary obstruction.
    • Previous cholecystectomy with common bile duct exploration for calculi can lead to retained stones or strictures.

    Observation:

    • Ultrasonic scanning of the right upper quadrant was performed in four patients with prior cholecystectomy and common bile duct exploration.
    • Clinical and laboratory data were insufficient to determine the exact nature of obstruction in these patients.

    Findings:

    • All patients showed biliary duct dilatation on ultrasound.
    • The majority of patients demonstrated bile duct calculi on ultrasound.
    • Ultrasonic scanning identified biliary duct dilatation and potential calculi in complex post-cholecystectomy cases.

    Implications:

    • Ultrasonic scanning is a valuable adjunct for diagnosing biliary tract obstruction when conventional methods are inconclusive or not feasible.
    • It should not replace standard cholangiography (oral, IV) or invasive procedures (ERCP, PTC, liver biopsy) when these are indicated and safe.
    • Ultrasound offers a non-invasive option for challenging diagnostic scenarios in biliary tract disease.
