Summary

Stomal recurrence after radical laryngeal surgery for advanced laryngeal cancer occurred in 7% of patients. Most recurrences involved neck metastasis, leading to a poor prognosis with a short survival time post-recurrence.

Area of Science:

  • Oncology
  • Surgical Oncology
  • Laryngology

Background:

  • Laryngeal carcinoma presents significant treatment challenges, particularly in advanced stages (III and IV).
  • Radical laryngeal surgery, including laryngectomy, is a primary treatment modality for these advanced cases.
  • Post-surgical complications, such as stomal recurrence, require careful monitoring and understanding.

Purpose of the Study:

  • To investigate the incidence and characteristics of stomal recurrence following radical laryngeal surgery.
  • To analyze the relationship between stomal recurrence and cervical nodal metastasis.
  • To determine the survival outcomes for patients experiencing stomal recurrence.

Main Methods:

  • Retrospective analysis of 71 patients who underwent radical laryngeal surgery for stage III and IV laryngeal carcinoma.

Related Experiment Videos

  • Follow-up focused on identifying and documenting stomal recurrence.
  • Analysis of patient data including time to recurrence, presence of nodal metastasis, and survival duration.
  • Main Results:

    • Stomal recurrence was observed in 5 patients (7%) out of 71.
    • Four of the five patients with stomal recurrence also had cervical nodal metastasis.
    • The average time from laryngectomy to stomal recurrence was 19 months; all affected patients expired, with a mean survival of 5.3 months post-recurrence.

    Conclusions:

    • Stomal recurrence is a significant complication in advanced laryngeal carcinoma patients undergoing radical surgery.
    • The presence of cervical nodal metastasis appears to be a critical factor associated with stomal recurrence.
    • Stomal recurrence carries a very poor prognosis, highlighting the need for vigilant surveillance and potentially improved surgical or adjuvant strategies.
