Background:

  • Sulfadiazine is a sulfonamide antibiotic used to treat bacterial infections.
  • Drug-induced hypersensitivity reactions can manifest in various organs, including salivary glands.

Observation:

  • A 50-year-old male with bronchitis developed acute parotid gland swelling and ear plugging within 2 hours of ingesting Bronco-aseptilex, a medication containing sulfadiazine.
  • Initial skin-prick tests with sulfadiazine and Bronco-aseptilex were negative.
  • Oral challenge tests with Bronco-aseptilex and subsequently sulfadiazine alone reproduced the salivary gland enlargement.

Findings:

  • The patient presented with symptoms consistent with sialadenitis.
  • Oral challenge testing definitively identified sulfadiazine as the causative agent for drug-induced sialadenitis.
  • Skin-prick tests were not indicative of this specific hypersensitivity reaction.

Implications:

  • This case highlights sulfadiazine as a potential cause of drug-induced sialadenitis.
  • Clinicians should consider drug-induced sialadenitis in patients presenting with acute salivary gland swelling, especially after sulfonamide use.
  • Further investigation into the mechanisms of sulfadiazine-induced salivary gland reactions may be warranted.
