Bone composition and vitamin D after Pólya gastrectomy

L Tougaard, H Rickers, P Rödbro

    Acta Medica Scandinavica
    |January 1, 1977
    PubMed
    Summary

Gastrectomized patients show biochemical signs of mild vitamin D insufficiency. The bone phosphorus/hydroxyproline ratio (P/Hypro) may indicate early osteomalacia in these individuals.

    Area of Science:

    • Biochemistry
    • Bone Metabolism
    • Endocrinology

    Background:

    • Gastric surgery, such as gastrectomy, can lead to malabsorption of nutrients crucial for bone health.
    • Osteomalacia, a condition of impaired bone mineralization, can occur secondary to vitamin D deficiency.
    • Assessing early bone changes in post-gastrectomy patients is important for timely intervention.

    Purpose of the Study:

    • To evaluate the bone phosphorus/hydroxyproline ratio (P/Hypro) as a potential index for detecting osteomalacia.
    • To investigate bone mineral content (BMC) and calcium metabolism in gastrectomized patients.
    • To correlate bone P/Hypro with biochemical markers of bone health and vitamin D status.

    Main Methods:

    • Analysis of bone P/Hypro and BMC in 27 gastrectomized patients without clinical bone symptoms.
    • Measurement of plasma calcium, plasma magnesium, serum parathyroid hormone (iPTH), plasma alkaline phosphatase, and serum 25-hydroxycholecalciferol (25-OH-D).
    • Comparison of biochemical indices between patients with and without regular vitamin D supplementation.

    Main Results:

    • Gastrectomized patients exhibited subnormal bone P/Hypro, BMC, plasma calcium, and plasma magnesium.
    • Elevated serum iPTH and plasma alkaline phosphatase levels were observed.
    • A significant positive correlation was found between bone P/Hypro and serum 25-OH-D, but not with BMC.
    • Patients without vitamin D supplementation had lower serum 25-OH-D and bone P/Hypro, and higher iPTH.

    Conclusions:

    • Gastrectomized patients demonstrate biochemical evidence of mild vitamin D insufficiency.
    • Low bone P/Hypro values in these patients suggest the presence of mild osteomalacic changes.
    • The bone P/Hypro ratio shows potential as an early indicator of osteomalacia in post-gastrectomy populations.
