The tonsillectomy-adenoidectomy dilemma

J A McCurdy

    Summary

Tonsillectomy and adenoidectomy (T&A) is a common surgery. While T&A may cause an apparent immunologic deficit, its clinical significance is unknown, as antibody responses and viral infections remain normal.

    Area of Science:

    • Otolaryngology
    • Immunology
    • Microbiology

    Background:

    • Tonsillectomy and adenoidectomy (T&A) is the most frequently performed surgical procedure in the United States.
    • Significant controversy exists regarding T&A due to methodological flaws in prior research.
    • The clinical relevance of the observed immunologic deficit following T&A remains undetermined.

    Purpose of the Study:

    • To evaluate the clinical significance of the immunologic deficit potentially caused by tonsillectomy and adenoidectomy.
    • To assess the impact of T&A on humoral antibody responses and susceptibility to viral infections.
    • To investigate the effect of T&A on pharyngeal microflora.

    Main Methods:

    • This study likely involved the assessment of immune responses and infection rates in patients undergoing T&A.
    • Methods probably included measuring humoral antibody levels and monitoring for clinical signs of viral infections.
    • Analysis of pharyngeal microflora before and after the procedure was likely conducted.

    Main Results:

    • Patients demonstrated normal humoral antibody responses post-surgery.
    • No increase in clinically apparent viral infections was observed after T&A.
    • T&A consistently resulted in the normalization of pharyngeal microflora.

    Conclusions:

    • The apparent immunologic deficit following T&A lacks clear clinical significance.
    • T&A does not appear to impair humoral immunity or increase susceptibility to viral infections.
    • T&A effectively normalizes the pharyngeal microflora, suggesting a potential benefit in managing upper airway conditions.
