    Summary

    Adnexal conservation during hysterectomy for benign conditions is generally safe. Subsequent re-exploration for adnexal pathology, mainly corpus luteum cysts, occurred in 1.4% of patients, with no malignancies found.

    Background:

    • Hysterectomy is a common surgical procedure for benign gynecologic conditions.
    • Adnexal conservation (ovaries and fallopian tubes) aims to preserve ovarian function in premenopausal women.

    Purpose of the Study:

    • To evaluate the incidence and nature of adnexal pathology requiring re-exploration after hysterectomy with adnexal conservation.
    • To assess the safety and outcomes of preserving adnexa in patients undergoing hysterectomy for benign diseases.

    Main Methods:

    • Retrospective analysis of 922 patients undergoing abdominal hysterectomy with at least one adnexum conserved for benign diseases.
    • Review of subsequent re-explorations for adnexal pathology.

    Main Results:

    • Thirteen patients (1.4%) required re-exploration for adnexal pathology.
    • Corpus luteum cysts were the most common finding (77% of re-explored cases).
    • Primary Fallopian tube pathology was identified in 38% of re-explored cases, with no ovarian malignancies detected.
