  • A systematic evaluation of every third prescribing error with potential clinical importance was conducted over one year in a tertiary care teaching hospital.
  • Errors were concurrently assessed for potential adverse consequences and retrospectively analyzed by physicians and pharmacists to identify related factors.

  • A total of 696 errors with potential for adverse effects were analyzed.
  • The most frequent specific factors included altered renal/hepatic function (13.9%), patient allergy history (12.1%), incorrect drug nomenclature (11.4%), dosage calculation errors (11.1%), and atypical dosage frequencies (10.8%).
  • Common contributing factor groups were related to knowledge of drug therapy (30%), patient factors affecting drug therapy (29.2%), calculation/expression errors (17.5%), and nomenclature issues (13.4%).

Conclusions:

  • Numerous identifiable factors are linked to a substantial proportion of medication prescribing errors.
  • Targeting educational, technological, and risk management efforts on these common factors can mitigate patient risk from adverse drug events.
