Summary

Chronic esophagitis dissecans is a newly identified esophageal condition characterized by dysphagia and mucosal shedding. This distinct clinicopathologic entity affects elderly patients and requires recognition for accurate diagnosis.

Area of Science:

  • Gastroenterology
  • Pathology
  • Cell Biology

Background:

  • A unique form of chronic esophagitis was observed.
  • This condition is proposed to be termed chronic esophagitis dissecans.

Purpose of the Study:

  • To describe the clinical and histologic features of chronic esophagitis dissecans.
  • To differentiate this entity from other causes of chronic dysphagia.

Main Methods:

  • Retrospective analysis of five patients diagnosed between 1988 and 1994.
  • Clinical, endoscopic, histologic, and ultrastructural examinations were performed.
  • Immunohistochemistry was used to assess cell adhesion molecules.

Main Results:

  • Patients were elderly (mean age 66) with chronic dysphagia and mucosal fragment shedding.

Related Experiment Videos

  • Esophageal strictures were present, but without reflux symptoms or concurrent skin lesions.
  • Histology revealed mucosal blistering with altered cell-cell adhesion, indicated by reduced desmosomes and E-cadherin expression.
  • Conclusions:

    • Chronic esophagitis dissecans is a previously unrecognized clinicopathologic entity.
    • This condition should be considered in the differential diagnosis of chronic dysphagia.
    • Altered intercellular adhesion is a key feature of this esophagitis.
