Summary

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease primarily affecting adults, with risk increasing with age and affecting men more than women. While genetics play a role in familial cases, environmental factors and toxicants may contribute to sporadic ALS.

Area of Science:

  • Neuroscience
  • Genetics
  • Epidemiology

Background:

  • Amyotrophic lateral sclerosis (ALS) is an adult-onset neurodegenerative disorder with unknown causes.
  • ALS incidence rises with age, and men are more frequently affected than women.
  • Genetic susceptibility is a key risk factor, implicated in approximately 10% of familial ALS cases.

Purpose of the Study:

  • To review the known risk factors and potential etiologies of Amyotrophic Lateral Sclerosis (ALS).
  • To explore the interplay between genetic predisposition and environmental influences in ALS development.
  • To summarize current understanding of ALS pathogenesis based on genetic and epidemiological evidence.

Main Methods:

  • Review of genetic linkage studies identifying mutations in genes like superoxide dismutase.
  • Analysis of epidemiological studies investigating occupational exposures and environmental toxicants.
  • Examination of evidence for other potential risk factors such as physical trauma and exertion.

Main Results:

  • A mutant form of the Cu/Zn superoxide dismutase gene is implicated in 15-20% of familial ALS.
  • Associations exist between sporadic ALS and occupational toxicant exposure.
  • Evidence for roles of physical trauma, electrical shock, and vigorous exertion in ALS etiology is inconsistent.

Conclusions:

  • While genetic factors are established in familial ALS, the etiology of sporadic ALS likely involves environmental toxicants acting on a susceptible background.
  • Further research is needed to elucidate the specific mechanisms linking environmental exposures and genetic susceptibility to ALS.
  • Understanding these factors is crucial for developing effective prevention and treatment strategies for ALS.
