Background:

  • Undifferentiated embryonal sarcoma of the liver (UESL) is a rare and aggressive malignant liver tumor predominantly affecting children.
  • Accurate diagnosis is crucial for effective management and improved patient outcomes.

Observation:

  • A 15-year-old male presented with a palpable right upper quadrant abdominal mass.
  • Initial T1-weighted MRI demonstrated a well-defined, hypointense left hepatic lobe mass with scattered high signal intensities.
  • T2-weighted imaging showed the mass becoming hyperintense, and postcontrast T1-weighted imaging revealed irregular enhancement of the solid components.

Findings:

  • MRI findings included a well-defined, heterogeneous liver mass with specific signal intensity characteristics on T1- and T2-weighted sequences.
