    Background:

    • Normal pressure hydrocephalus (NPH) is a condition characterized by dementia, gait disturbance, and urinary incontinence.
    • Traditionally, NPH is treated with cerebrospinal fluid (CSF) shunting, but its efficacy requires further investigation.

    Observation:

    • A patient presented with classic NPH symptoms: dementia, incontinence, and apraxic gait.
    • Diagnostic imaging revealed hydrocephalus with absent air over the cerebral convexities and delayed contrast clearance.
    • Lumbar CSF pressure was within the normal range.

    Findings:

    • The patient experienced spontaneous improvement in cognitive function over several years without receiving a shunt.
    • This outcome challenges the assumption that shunting is always necessary for NPH.

    Implications:

    • The findings underscore the importance of conducting controlled studies to determine the optimal treatment strategy for NPH.
    • Further research is needed to identify predictors of spontaneous recovery and refine indications for shunting.
