Background:

Hepatitis B virus (HBV) infection remains a significant global health concern.
Identifying key transmission routes is essential for effective prevention strategies.

Purpose of the Study:

To investigate the risk factors associated with acute hepatitis B infection in Naples.
To inform public health interventions for HBV prevention.

Main Methods:

A case-control study was conducted between 1989-1991.
162 acute hepatitis B cases were compared with 788 hospitalized controls.
Multivariate analysis was employed to identify independent risk factors.

Main Results:

Surgical intervention (OR 3.8), household contact with an HBsAg-positive carrier (OR 2.7), and intravenous drug use (OR 13.0) were significantly associated with hepatitis B.

No association was found with blood transfusion, hospitalization, other percutaneous exposures, dental therapy, or sexual history.

Conclusions:

Surgical procedures and household contact with carriers represent significant HBV transmission pathways.
Enhanced sterilization and use of disposable materials in surgical settings are recommended to curb HBV transmission.
