Summary

Tertiary hyperparathyroidism, a condition resistant to treatment, can occur after kidney transplants. Subtotal parathyroidectomy effectively normalized metabolic parameters in three young patients, suggesting aggressive management of secondary hyperparathyroidism is key.

Background:

Tertiary hyperparathyroidism is characterized by autonomous parathyroid gland proliferation and hyperfunction, often resistant to standard therapies.
It is hypothesized to result from the inactivation of a tumor suppressor gene on chromosome 11.
This condition can develop in patients with chronic renal insufficiency, even after successful renal transplantation.
