Background:

  • Progressive multifocal leukoencephalopathy (PML) is a rare, serious demyelinating disease affecting the brain.
  • Accurate and timely diagnosis of PML is crucial for patient management.

Observation:

  • Serial MRI and magnetization transfer imaging (MTI) were performed on a patient with biopsy-proven PML.
  • Magnetization transfer ratio (MTR) values were assessed and compared to healthy controls.
  • The spatial distribution of MTR changes was analyzed.

Findings:

  • The patient with PML exhibited a profoundly and significantly diminished MTR compared to normal controls.
