    Cell-mediated immunity (CMI) to sperm is common in fertile and infertile individuals, including newborns. This suggests sperm CMI may play a role in fertility, not just infertility diagnosis.

    Background:

    • Previous studies on antisperm cell-mediated immunity (CMI) were limited by leukocyte contamination in sperm preparations.
    • Developing pure sperm antigens is crucial for accurate CMI assessment.

    Purpose of the Study:

    • To investigate cellular immunity to spermatozoa in various human populations using purified sperm and lymphocyte proliferation assays.
    • To determine the diagnostic utility of sperm CMI in immunological infertility.

    Main Methods:

    • Isolated pure, viable spermatozoa using Percoll gradients.
    • Used sonicated/cavitated sperm extracts and live spermatozoa.
    • Assessed lymphocyte proliferation via thymidine incorporation assay.

    Main Results:

    • Significant lymphocyte proliferation to sperm extracts observed in vasectomized men (69%), fertile men (50%), infertile women (27%), and fertile women (50%).
    • Viable sperm induced lymphocyte responses in fertile women (63%), healthy men (63%), and cord blood (45%).
    • No correlation found between serum antisperm antibodies and sperm CMI.

    Conclusions:

    • Lymphocyte proliferation to spermatozoa is prevalent across fertile, infertile, and even sperm-naive newborn populations.
    • The lymphocyte proliferation assay is not a reliable diagnostic tool for immunological infertility.
    • Immunological recognition of spermatozoa is common and may influence fertility.
