Acipimox provides comparable image quality and FDG clearance for myocardial viability assessment using FDG SPET as hyperinsulinaemic glucose clamping. This method may be useful in clinical routine for assessing cardiac viability.

  • Fluorine-18 fluorodeoxyglucose (FDG) Positron Emission Tomography (PET) is used to assess myocardial viability.
  • FDG imaging with Single-Photon Emission Tomography (SPET) has been described.
  • Optimal image quality for cardiac FDG studies typically requires hyperinsulinaemic glucose clamping.

Purpose of the Study:

  • To compare the image quality and FDG clearance of cardiac FDG SPET studies.
  • To evaluate FDG SPET after oral glucose loading, acipimox administration, and hyperinsulinaemic glucose clamping.

Main Methods:

  • Eight patients underwent cardiac FDG SPET studies.
  • Image quality was assessed using the myocardial to blood pool (M/B) activity ratio.
  • FDG plasma clearance was measured by blood sampling and gamma well counting.

  • M/B ratios were comparable between clamping (2.8+/-0.8) and acipimox (2.9+/-0.7), but lower after oral glucose loading (2.2+/-0.3).

  • Acipimox showed comparable clearance rates to clamping and significantly faster clearance than oral glucose loading.

  • FDG SPET imaging after acipimox administration yields image quality comparable to hyperinsulinaemic glucose clamping.
  • FDG SPET with acipimox demonstrates clearance rates similar to clamping.
  • FDG SPET in combination with acipimox is a potentially useful method for routine clinical assessment of myocardial viability.
