    Background:

    • Adenomyosis is a common gynecologic condition.
    • Its etiology remains largely unknown.
    • It often presents with non-specific symptoms and coexists with other pelvic pathologies.

    Purpose of the Study:

    • To determine the prevalence of adenomyosis in hysterectomy specimens.
    • To identify associated pelvic pathologies.
    • To evaluate the clinical recognition of adenomyosis.

    Main Methods:

    • Retrospective analysis of 1619 consecutive hysterectomy specimens.
    • Histopathologic examination to identify adenomyosis and other findings.
    • Review of clinical data, including patient demographics, symptoms, and surgical observations.

    Main Results:

    • Adenomyosis was identified in 161 (9.9%) of the hysterectomy specimens.
    • It coexisted with other pelvic pathologies in 97 cases (60.2%).
    • Commonly associated conditions included leiomyomata, endometrial hyperplasia/carcinoma, and endometriosis.
    • Preoperative diagnosis was suspected in only 10% of women, and it was unrecognized surgically in 65% of cases.

    Conclusions:

    • Adenomyosis is a prevalent condition often associated with other gynecologic diseases.
    • Its uncharacteristic clinical presentation and frequent coexistence with other pathologies contribute to its underdiagnosis.
    • Improved awareness and diagnostic strategies are needed for this neglected condition.
