Aortic and peripheral blood pressure during isometric and dynamic exercise

Summary

Peripheral blood pressure measurements (Riva-Rocci/Korotkov and Finapres) significantly overestimate cardiovascular load during dynamic exercise compared to direct aortic measurements. This suggests current exercise testing blood pressure criteria need re-evaluation.

Area of Science:

  • Cardiovascular Physiology
  • Exercise Science
  • Medical Instrumentation

Background:

  • Accurate blood pressure monitoring is crucial for assessing cardiovascular load during exercise.
  • Peripheral blood pressure measurements are commonly used but may differ from central aortic pressure.

Purpose of the Study:

  • To compare direct aortic blood pressure (AOR) with peripheral measurements (Riva-Rocci/Korotkov [RRK] and Finapres [FIN]) during static and dynamic exercise.
  • To evaluate the accuracy of peripheral methods in reflecting cardiovascular stress during exercise.

Main Methods:

  • Direct aortic pressure measurement via an introduced tip manometer in 7 cardiac patients.
  • Comparison with simultaneous peripheral RRK and FIN measurements during static and maximal cycle ergometry.

Related Experiment Videos

Main Results:

  • Peripheral RRK and FIN methods showed significantly larger increases in systolic pressure than AOR during dynamic exercise, often doubling the observed rise.
  • Rate-pressure product calculations indicated RRK and FIN overestimated cardiovascular load by 197% and 181%, respectively, compared to AOR's 133% increase.

Conclusions:

  • Riva-Rocci/Korotkov and Finapres methods greatly overestimate cardiovascular system load during dynamic exercise.
  • Current criteria for blood pressure assessment in exercise testing require critical review due to discrepancies with central aortic pressure.
